Please use the following forms to submit your requests to the Animal Care Unit.
Animal Order Request
All acquisition and procurement of vertebrate animals, regardless of species or housing location, must be specifically approved in advance by the IACUC via an approved Animal Use Statement and the University Attending Veterinarian or designee. To facilitate approval and to maintain appropriate records, the online Animal Order Request Form must be completed and submitted. For deliveries from approved ACU rodent and rabbit vendors, except the National Institute of Aging (NIA), Animal Order Requests must be submitted by noon on Thursday for delivery the following week. NIA orders must be submitted by noon on Monday for delivery the following week. Links to access rodent and rabbit vendor catalogs are provided below for your information and to assist in completion of the Animal Order Request Form.

National Institute of Aging
Drugs, Supplies, and Equipment Order Form
Complete this form to initiate an order for pharmaceutical grade drugs, supplies, or equipment for animal use. The ACU does not maintain stock inventory. Orders should be placed in advance of need to allow for variable delivery depending on vendor availability and shipping arrangements. A 20% procurement surcharge will be assessed for purchase of all drugs, supplies, and equipment ordered through the ACU.
Animal Report Form (pdf)
This form serves to initiate a request for veterinary care and must be completed when unexpected clinical signs are observed in a research animal. Completed forms should be faxed to 785-864-5305.
Animal Transport/Transfer Request (pdf)
All movement of or transfer of animals between Animal Use Statements, regardless of species or housing location, must be specifically approved in advance by the IACUC via an approved Animal Use Statement and the University Attending Veterinarian or designee. To facilitate approval and to maintain appropriate records this pdf form must be completed and submitting to the Animal Care Unit. Transport requests must be received 48 hours prior to date of requested transport.
Non-Approved Vendor Source Rodent Request (pdf) and Rodent History Questionnaire (pdf)
Receipt of rodents from non-commercial sources must be approved in advance by the University Attending Veterinarian or designee. The Non-Approved Vendor Source Rodent Request and Rodent History Questionnaire forms, along with the Animal Order Form must be completed to initiate importation of animals from non-approved sources. Completed forms should be faxed to 785-864-5305.
Technical Services Request
ACU personnel are available to provide technical assistance on a fee-for-service basis. Examples of services include: animal identification, breeding colony management, anesthetic and surgical support, sample collection, and euthanasia. Complete this form to initiate a request for technical services. Technical service requests must be submitted 48 hours in advance of service request date.
